
The Building Blocks to An Effective Community Health Worker Program, Part 1: First Steps On-Demand
In Part 1 of this two Part webinar series, the roles that Community Health Workers (CHWs) play in enhancing healthcare delivery will be defined. A funding analysis will be shared, examples of successful programs will be discussed, and the first steps to designing a CHW program will be outlined.

The AAFP has reviewed this Program Development and Performance Improvement Strategies to Improve Patient Engagement and Safety training and deemed it acceptable for AAFP credit. The term of approval is for one year. Participants should claim only the credit commensurate with the extent of their participation in the activity.
This live session is approved for 0.75 AAFP Elective credits. Elective credits are available for physicians, physician assistants, nurses, nurse practitioners, and medical assistants. Attendance at the entire live training and the submission of a post-training evaluation are required to receive credit.
On-Demand training is not eligible for CMEs.

Jenene R. Washington, MD, MBA, FAAP

Dr. Jenene Washington is a physician executive who has over twenty (20) years of healthcare experience and over fifteen (15) years of success in practice management, practice transformation and operational excellence of healthcare organizations.
Dr. Washington has a passion for serving underserved communities and has worked as a practicing pediatrician and clinical leader in Federally Qualified Health Centers. She has held roles Chief of Pediatrics, Associate Chief Medical Officer, Director of Patient Centered Services, and Chief Medical Officer. In these roles, she managed multidisciplinary medical practices that included primary care specialties, obstetrics/gynecology, School-based health, and behavioral health.
Dr. Washington is the Founder and Chief Executive Officer of Renaye James Healthcare Advisors, a boutique healthcare consultant group that works with healthcare practices and partners to transform them into high performing, high quality organizations. Her team implements new programs, obtains funding, attains accreditations, improves care processes to help organizations realize gains in revenue, quality, and patient loyalty.
Dr. Washington has degrees in Chemistry (BS), Medicine (MD), and Medical Services Management (MBA). She is a certified Project Management Professional (PMP), a PCMH – Certified Content Expert and Lean/Six Sigma Green-Belt Trained.

Muhammed Mamman, MBA, MHA, is a seasoned healthcare operations leader with 15+ years of experience improving care delivery across primary care, urgent care, community-based services, and program development. Known for driving operational excellence, he specializes in patient-centered care models that enhance access, improve outcomes, and elevate patient satisfaction. His expertise spans healthcare operations, project management, fiscal oversight, and quality improvement.
As Senior Director of Patient/Client Access at Health Care for the Homeless in Baltimore, Muhammed advances care for underserved populations while leading initiatives that strengthen financial performance, operational efficiency, and workforce development. He has extensive experience training clinical and operational staff, facilitating practice transformation, and conducting learning collaboratives on SBIRT, Motivational Interviewing, Substance Use Disorder, and HIV/Hepatitis C. His contributions also include supporting crisis center assessment and implementation projects in Southern Maryland and St. Mary’s County.
Muhammed is a strong advocate for patient safety and quality improvement, employing Root Cause Analysis, interdisciplinary rounding, team training, and leadership walk rounds to foster a culture of safety. He serves on the National Health Care for the Homeless Council’s administrative committee, collaborating nationally on solutions at the intersection of healthcare and homelessness.
He previously served as a Care Transformation Organization Director for the Maryland Primary Care Program (MDPCP), a CMS initiative aimed at reducing healthcare costs and improving quality. He has implemented sustainable, cost-effective care coordination processes across multiple care settings and contributes operational expertise to a UnitedHealthcare project focused on evidence-based improvements in clinical quality, population health, and revenue cycle management. He also managed COVID-19 testing and vaccination programs in Baltimore.
Throughout his career, Muhammed has led initiatives that expand access to care, improve patient experience, and reduce costs, including operational roles at Righttime Medical Care (now MedStar Health Urgent Care) and the development of extended-hours programs at Baltimore Medical System.
Muhammed holds a Master of Business Administration (MBA) and a Master of Health Care Administration (MHA) from the University of Maryland Global Campus.
