The Building Blocks to an Effective Community Health Worker Program, Part 2: Monitoring Success (On-demand)

In Part 2 of this two part webinar series, presenters will review the steps to design an effective Community Health Worker (CHW) program; provide tools to plan, launch, and maintain the program; review CHW training needs; and suggest ways to monitor program success. CHWs are essential healthcare team members who work to educate and coordinate patient care, and this training will set the stage for their success.

AAFP Credit System Seal

The AAFP has reviewed this Program Development and Performance Improvement Strategies to Improve Patient Engagement and Safety training and deemed it acceptable for AAFP credit. The term of approval is for one year. Participants should claim only the credit commensurate with the extent of their participation in the activity.

This live session is approved for 0.75 AAFP Elective credits. Elective credits are available for physicians, physician assistants, nurses, nurse practitioners, and medical assistants. Attendance at the entire live training and the submission of a post-training evaluation are required to receive credit.

On-Demand training is not eligible for CMEs.

Her team implements new programs, obtains funding, attains accreditations, improves care processes to help organizations realize gains in revenue, quality, and patient loyalty.

Dr. Washington has degrees in Chemistry (BS), Medicine (MD), and Medical Services Management (MBA). She is a certified Project Management Professional (PMP), a PCMH – Certified Content Expert and Lean/Six Sigma Green-Belt Trained.
