NTTAP CoP: Improve Your Depression Screening for Health Center Populations

This Community of Practice introduces participants to common depression and other mental health screening tools that are available to the health center population, and it relays strategies to implement these tools into the health center’s standard operating procedures. This Community of Practice will provide steps on how to effectively collaborate with mental health providers to assist in managing depression and other co-occurring mental health conditions with a focus on school-based health, maternal health, and older adult patients.

The AAFP has reviewed this Strategies for Preventive Health and Chronic Disease in Maternal, Pediatric, Geriatric, and Adult Care training and deemed it acceptable for AAFP credit. The term of approval is for one year. Participants should claim only the credit commensurate with the extent of their participation in the activity.

Each live session is approved for 1.25 AAFP Elective credits. Elective credits are available for physicians, physician assistants, nurses, nurse practitioners, and medical assistants. Attendance at the entire live training and the submission of a post-training evaluation are required to receive credit.

Presenters

Christina Clayton, MS, BSN, RN

Director, Clinical Services, Renaye James Healthcare Advisors

As a dedicated healthcare leader, Christina has over 13 years’ experience as a nurse leader in community, hospital, health insurance, and regulatory settings.

Christina has had a valuable impact on healthcare quality improvement. She has had positions in nursing leadership, nursing management, case management, payer quality programs, program management, regulatory compliance, and nursing education.

Prior to Renaye James, as a Centers for Medicare & Medicaid Services (CMS) nurse consultant, Christina provided accreditation oversight to Medicare’s acute care programs. She served as the program lead for Diabetes Self-Management Training and Home Infusion Therapy accreditation programs. Additionally, she was the clinical support for Rural Health and Federally Qualified Health Center programs. Christina provided oversight to national accreditation organizations and provides direction in the development of education material for state surveyors for those programs. Additionally, she was responsible for collaborating with other federal agencies and national associations.

As a program manager for Johns Hopkins Healthcare, LLC, Christina aligned measurable program goals with organization’s mission, and utilized quality improvement strategies and measurement to implement changes the health plan’s programs. Christina has been instrumental in improving the health of the community; as a Program Manager, she collaborated with external stakeholders to improve health outcomes and decrease high utilization of members within the Complex Case Management and Disease Management Program.

As a FQHC Clinical Manager, she has vast experience in health center operations, care management, health center budgeting, nursing management, and medical assistant supervision.

Christina has a Bachelor of Science in Nursing from Coppin State University (Baltimore, Maryland) and a Master of Science in Nursing Administration from Morgan State University (Baltimore, Maryland).

Carol Ivy Simmons, PhD, LCSW-C

Senior Advisor, Behavioral Health Services, Renaye James Healthcare Advisors

Carol Ivy Simmons, PhD, LCSW-C, brings more than 20 years of leadership and clinical experience to the field of behavioral health. She has served in key clinical and administrative roles across a broad range of settings, including inpatient and outpatient programs, health centers, substance use disorder services, and crisis response systems. Dr. Simmons has also held prominent public-sector positions, serving as a Federal Liaison Officer for the Health Resources and Services Administration (HRSA) and as a State Commissioner for the Community Health Resource and Services Commission (CHRSC).

Throughout her career, Dr. Simmons has been recognized as a transformational leader who leverages data to drive outcomes, applies systems theory to identify strengths and opportunities, expands access to care, establishes high-quality performance metrics, and ensures strong fiscal stewardship.

Dr. Simmons earned a master’s degree in social work—with concentrations in clinical practice and administration—from Hunter College School of Social Work in New York. She holds a PhD in Advanced Behavioral Health Science and Leadership from Capella University. She is licensed in the District of Columbia, Maryland, and Virginia, and is a Board-Certified Supervisor in Maryland.

Vivian D. Echevarria Guzman, LPC-S

 Senior Director, School-Based Therapy, Legacy Community Health

Vivian Echevarria Guzman has over a decade of experience in community mental health and serves as the Senior Director of School-Based Therapy at Legacy Community Health. In this role, she leads strategic partnerships with school district leaders and operations teams from KIPP, Yes Prep, and Galena Park. Vivian oversees the performance and clinical development of over 40 clinicians across school-based clinics and champions the use of group therapy to expand access and improve care outcomes.

Vivian began her career at Legacy as a counseling therapist in 2013, where she collaborated with fellows to support youth through groups focused on autism, self-esteem, and attention-deficit/hyperactivity disorder. Over the years, she has developed and implemented a wide range of therapeutic modalities, including individual and group therapy, behavior management, conflict resolution, parenting training, and trauma-focused treatment. Her clinical approach integrates Nature Informed Therapy, Acceptance and Commitment Therapy (ACT), and Dialectical Behavior Therapy (DBT), with a special focus on high school students impacted by immigration-related family separation.

A proud wellness advocate, Vivian also facilitates adventure therapy retreats and leads Laughter Yoga sessions for military families, promoting healing through joy, movement, and connection. Her commitment to equity and holistic care is rooted in her early volunteer work with the American Red Cross Puerto Rico chapter, her time as a Social Skills Art Teacher at CREARTE, Inc., and her mentorship at the University of Puerto Rico. Outside of work, Vivian enjoys nature adventures with her husband and their two children, especially water-related activities that bring the family together.

Vivian earned her Bachelor of Studies in Fine Arts from the University of Puerto Rico, Río Piedras Campus, and her Master of Science in Community Counseling from the University of Phoenix, Southern Arizona Campus. Vivian is a Licensed Professional Counselor Supervisor and has completed a Language Immersion Exchange with the Universiade Federal de Minas Gerais in Brazil. She maintains professional memberships with both the American Counseling Association and the Puerto Rico Counseling Association.

José Rosado, LCPC, NCC, MBA

 Neurodivergent Therapist/Owner, Grey Matter Counseling & Consulting

José Rosado is a Licensed Clinical Professional Counselor with a dynamic career spanning clinical practice, nonprofit leadership, education, and creative advocacy. With experience as a therapist, executive director, peer recovery specialist, operations administrator, and adjunct professor, he brings a multidisciplinary approach to mental health and human services.

José’s work has ranged from overseeing multimillion-dollar federal grants at the Maryland Department of Health to building therapy programs through his private practice, where he specializes in psychotherapy, mentorship, and ketamine-assisted psychotherapy. He has led workforce development initiatives, managed statewide recovery efforts, and launched community-focused programs across both public and private sectors.

Beyond the clinical realm, José serves as adjunct faculty at Towson University in the Psychology Department and previously served as participating faculty at the University of Baltimore in the Merrick School of Business. José has created award-nominated podcasts and developed digital platforms to raise awareness about mental health in the creative community. He is deeply committed to champion issues, organizational growth, and breaking the stigma around mental health through innovative and inclusive work.

Whether leading on the frontlines of care or behind the scenes in strategy and operations, José is driven by purpose, creativity, and a belief in doing meaningful work that leaves a lasting impact. He received his master’s degree in clinical Mental Health Counseling from Northwestern University and his MBA and bachelor’s degree from Iona University and is an ADHD-Certified Clinical Services Provider specializing in serving neurodivergent populations.

Antoinette Wilson, PhD, LPC

Licensed Professional Counselor and Therapy Director, School-Based Health Care, Legacy Community Health

Dr. Antoinette Wilson is a Licensed Professional Counselor and Therapy Director for School-Based Health Care at Legacy Community Health. She began her career with Legacy as a KIPP Therapist and advanced to her current leadership role, where she oversees hiring for school-based therapy positions and provides individual and family therapy services. Dr. Wilson is passionate about helping children have a safe space to express their emotions and supporting them on their healing journey.

She has delivered numerous presentations and workshops to professional audiences on topics including stress management, sexual assault prevention, and suicide prevention. Dr. Wilson has also lectured at Sam Houston State University. She has published and presented research papers at the Southwest Educational Research Association's annual meeting.

She is a proud member of Alpha Kappa Alpha Sorority, Inc., and has been happily married for over a decade. She loves being a “girl mom” to her daughter.

Dr. Wilson earned a Bachelor of Arts degree in Psychology from the University of North Texas, a Master of Arts degree in Counseling from Midwestern State University, and a Doctorate in Counselor Education at Sam Houston State University.

Arielle Mather, MPH

 Senior Program Manager, National Center for Equitable Care for Elders

Arielle Mather serves as Senior Program Manager at the National Center for Equitable Care for Elders, overseeing the planning and delivery of the Center’s training and technical assistance activities. She has spent many years in the Boston nonprofit sector addressing the needs of older adults, as well as the providers who care for them. Arielle received her Master of Public Health from Tufts University School of Medicine, where she had a concentration in health communication. Throughout her professional experience, she has enjoyed connecting others with resources that promote their health and well-being.
